A Clinical Roport on two Cases of Essential Thrombocythemia

Abstract
Two cases of essential thrombocythemia of a 64 year old Korean housewife and a 44 year old korean farmer were presented.
The first case was admitted with hematemesis, melena and previously history of severe epistaxis.
On physical examination, the conjunctiva was pale, liver was 2FB palpable in the epigastriun and was firm, smooth and non tender with sharp edge, and tip of spleen was also palpable.
Laboratory examinations disclosed anemia(Hb;8.0), leucocytosis (over 10,000) with neutrophilia, marked thrombocythemia (1, 610, 000/mm3), abnormal platelet aggragation with ADP and epinephrine, elevated LAP score and increased megakaryocytes with large bizzare shaped platelet clumps in bone marrow. X-ray study revealed no abnormality on UGI and esophagography.
The second case was admitted with left leg and abdominal pain.
On physical examination, the conjuntiva was anemic and swelling and tenderness were noticed on the left leg. Laboratory examinations disolosed anemia (Hb 7. 7), marked thrombocythemia (2,338,000/ mm3), increased megakaryocyte with large bizzare shaped platelet clumps in bone marrow but platelet aggregation with ADP and epinnephrine was within normal limit. Hypercalcemia, hyperkalemia and increased acid phosphatase were also noticed. UGI revealed gartric ulcer, and venography showed multiple thrombi in the femoral,¢¥ popliteal and external iliac vein. These two cases may be the first report of essential thrombocythemia in Korea which were confir¡þmed by the platelet function test together with review of the literature.
